All Seasons Buddy Bass Parent / Guardian Release Form
I have read the entry form and understand that
he or she will be fishing with an adult that is 18 years of age or older that is
not his or her parent or legal guardian in All Seasons Buddy Bass Tournament and
they are bound to same rules as all other contestants. In signing this
application I hereby give my consent for my child whom I am the parent or legal
guardian of to compete in All Seasons Buddy Bass Tournamnet and I waive and
release all other contestants, tournament sponsors and tournament officials from
all claims of injury or damage incurred in connection with All Seasons Buddy
Bass Tournament in anyway or form. He or she will abide and comply with the
rules and regulations of All Seasons Buddy Bass entry form.
